You pay one predictable fee, your preventive care is included, and you save 20% on qualifying treatment. There are no waiting periods, no deductibles, no claim forms and no annual maximums.

Membership Agreement Terms
- 12-month agreement: All plans are a full one-year commitment from your enrollment date.
- Automatic renewal: Plans renew for another 12 months unless you cancel in writing before your renewal date.
- No early cancellation: If you leave before your term ends, the remaining monthly fees through the end of your contract year are due.
- Written notice required: Verbal cancellations are not accepted. We recommend giving notice at least 30 days before renewal.
- No retroactive benefits: Benefits and discounts apply only to care received on or after your enrollment date.
- Not insurance: This is an in-house membership plan offered directly by our practice. It is non-transferable and for individual use only.
- Unused cleanings do not roll over to the next year.

Is this insurance? No.
This is an in-house membership plan offered directly by our practice. There are no claims, no networks and no surprise denials. Smile Plus is designed to work alongside your insurance, not replace it.
Are these plans a commitment?
Yes. All three plans are a 12-month agreement, either paid up front or billed monthly.
What happens when my year is up?
Your membership automatically renews for another 12 months unless you notify our office in writing before your renewal date.
Can I cancel early?
There is no early cancellation within the 12-month term. If you choose to end your membership early, the remaining monthly fees through the end of your contract year are due before the membership is released.
Is there a waiting period?
No. All three plans start on your enrollment date.
Can I apply the discount to treatment I received before enrolling?
No. Benefits apply only to care received on or after your enrollment date.
I have insurance. Why would I need Smile Plus?
Most insurance plans cover only 2 cleanings a year and have an annual maximum. Once that maximum runs out, you pay full price. Smile Plus adds 2 more cleanings per year and gives you 20% off treatment after your maximum is reached.
Can I use Smile Basic or Perio+ if I have insurance?
No. Those plans are for patients without dental insurance. If you have insurance, Smile Plus is the plan designed for you.
What is excluded from the 20% discount?
Invisalign is excluded on all three plans, and specialty referrals are not included. The discount applies to qualifying in-office treatment at the time of service.
Can I switch from Smile Basic to Smile Perio+?
Yes. If your gum health changes, we will help you move to the plan that fits.
Does the plan cover my family?
Each family member enrolls individually. Ask our front desk about enrolling everyone at the same time.
What if a monthly payment fails?
We will try the charge again and ask you to update your payment information. Membership benefits may be paused until the payment is resolved.
